Reminder, our cancellation policy is as follows: No shows may be charged the full session fee. Cancellations less than 48 hours from the appointment time are charged the full session fee. - Update Payment – Update your payment information via phone, email, or the update payment link
- Medication Requests and Prescription Refills – Patients may call or email the office/provider and leave a message for prescription refills. Refills are called in on Monday through Friday only, during normal business hours. Please allow 48 business hours for all requests. Our office takes the prescription of controlled medications very seriously. An initial face-to-face appointment AND an in-office visit every 90 days may be required by your provider, the DEA and/or the state where you reside. We will not make exceptions to local or federal regulations. If you are prescribed a controlled substance, appointments will be required for refills. Lost or stolen controlled substance prescriptions will not be replaced under any circumstances. If patients are having withdrawal symptoms due to lost or stolen controlled prescriptions, they are responsible for going immediately to the ER or calling 911 to seek immediate medical attention (i.e. we will not break our controlled medication policies because a patient states that they are having withdrawal).
Returning
- Contact us – Use our contact page to let us know you’re returning. Depending on your time away from the office, you may need to recomplete intake forms.
- Schedule appointments – depending on the length of time you’ve been away from our office, you will be scheduled for either an intake appointment or follow up appointment (this may require paperwork to be completed)
- Update Payment – Update your payment information via phone, email, or the update payment link
